Medication Management

Many people experience difficulty ordering, understanding, and remembering when and how to take their different prescriptions. That’s why Personal Care offers our convenient Prescription Management Program. One of our Registered Nurses will assist you in:

• Setting up medications
• Filling existing prescriptions
• Verifying and filling new prescriptions
• Communicating with your doctors’ offices
• Monitoring a client’s response to medication(s)
• Medication reminders
• Assistance with taking medications

Our Nurse Manager will set up the medications monthly, and document the medication plan as prescribed by the client’s doctor(s). The daily reminders and assistance will be handled by a Personal Care Certified Nursing Assistant or L.P.N.. Our Nurse Manager will monitor the medication plan through follow-up visits.
