Step 1: Tell Us a Little About You
Complete our contact form or call us anytime to discuss your needs and learn how we can support you or your loved one. During this call we will gather general information like name, address, and contact information, current health status of your loved one, information related to both their physical needs and their cognitive abilities, and your thoughts regarding the amount of care needed.
Step 2: Free Care Consultation
Meeting you or your loved one in person and seeing their living environment firsthand provides valuable insight for developing a successful care plan. The purpose of this visit is to establish a positive connection with you and involved family members, and collaborate to clearly define the specific help you need from our caregivers.
Step 3: Personalized Care Plan and Caregiver Matching
Following the visit, we will create your tailored care plan and your family will be assigned a Case Coordinator who will oversee your schedule and caregiver matching. Step 4: Begin Care Services and Continuous Support
Your Case Coordinator will regularly monitor caregiver notes, feedback, and communicate pertinent updates to you. As there are changes in health status or care needs, our clinical team will collaborate with you and involved family members to make any adjustments needed. All involved parties will have access to our portal where you can view assigned caregivers, care notes, calendars, care plans, and invoices, giving you insights on the care provided wherever you are.
